Waiver Section
I am aware that playing volleyball and being involved in the Camp Dig This Volleyball Camp involves many risks, dangers and hazards and carries with it the potential for death, serious injury, or property loss. I am aware of those risks and I HEARBY ASSUME THE RISKS OF PARTICIPATING IN THE CAMP DIG THIS VOLLEBALL CAMP.
I waive, release, absolve, indemnify and agree to hold harmless; the Camp Dig This Volleyball Camp; Golden Ticket Sports Centre; and any of it’s or their respective officers, directors, employees, volunteers, agents or any other representatives of the Camp Dig This Volleyball Camp against any and all causes of action, claims, demands, losses, expenses, ability. I further agree no to sue or make claim of any nature whatsoever relating to or arising out of my participation in any Camp Dig This Volleyball Camp or use of Facilities in any court, agency, or other forum or proceeding against any individual or entity who I have released and agreed to hold harmless in the preceding sentence.
Permission to Play (for Participants under 19 years of age)
I confirm that I am the legal guardian of the “athlete” and that by my signature I am consenting to his/her participation in training, competition, events and activities associated with the Camp Dig This Volleyball Camp and his/her uses of Golden Ticket athletic facilities. I understand that participation in the Camp Dig This Volleyball Camp is voluntary and at our own risk. I approve of the leaders who will be in charge of this camp and I recognize that the leaders are serving to the best of their ability. I certify to the best of my knowledge that the participant is physically fit to engage in the activities described above. I also agree to indemnify and hold harmless the persons or entities named in the Waiver and Release of any claims or liabilities assessed them. Give consent that all photographs taken during The Camp Dig This Volleyball Camp, practices can be used for promotional and advertising purposes whether it be print, website, radio, television, or any other forms of public.
I have read and understand this agreement and I am aware that by this agreement I am waiving certain legal rights which I or my heirs, next of kin, executors, administrators and representatives may have against the releasees.
